In order to register for any event, you will need to provide a current USA Hockey registration number.  In addition, you will be able to upload a copy of a current Consent to Treat and Code of Conduct (both USA Hockey forms).  To make your registration easier, have these documents completed, scanned, and saved so you can easily upload them.  The documents are linked below (BOTTOM OF THIS PAGE).

You DO NOT need your USA Hockey number or any other forms to make a regular season payment.
